The Billing Specialists are happy to assist you with any questions you may have about your insurance coverage or billing matters. Prior to your initial visit, we will verify your insurance and inform you of your complete benefits package. You can find a complete list of insurance companies here.

Step 1 – The Verification Process

DASHA® “independent client providers” accept a variety of payment methods.

Start by completing our Insurance Verification Form.

Once your insurance plan has been verified a Benefits Coordinator from the “Billing Company” will contact you and provide you with a verbal review of your insurance coverage. This breakdown  will include therapies, copays and deductibles.

Step 2 – Scheduling Your First Appointment

Once your insurance has been verified, schedule your first appointment with our Concierge Team

Step 3 – Welcome to the DASHA® “independent client providers”

Upon your first appointment, you will be given a copy of the reviewed insurance coverage in a Client Welcome Kit. At this point, please advise either the Concierge or the “Billing Company” of any changes to your insurance plan.

In order for us to provide the best care for you, it is very important that you bring the following items to your first visit:

  • Photo ID or License
  • X-rays
  • Insurance card
  • Medical records
  • Last blood workup
